Liver Function Test Using 99m Tc DISIDA





Abstract
It has been widely accepted notion that treatment of choice of liver tumors are hepatic resection. Difficult problem in liver resection for malignancy can be attributed to one reason, mainly, uncertainty of remaining hepatic function. Due to the fact of coexistence of cirrhosis and hepatoma in majority of cases, accurate evaluation of hepatic function pre-and post-operatively seems to play key role for successful hepatic resection.
Many tests have been used to determine hepatic function up to date. Among of these, Indocyanine Green Maximal Removal Rate (ICG R-max) and its plasma disappearance rate (ICG-K) have been most frequently used. Because of time consuming process and burden for patient, measurement of ICG R-max has not been widely used. Determination of ICG-K is simple and effective way to measure hepatic function, but it can not show an exact picture of cirrhotic and space occupying area.
To accomplish those two objects, this experiment is carried out using with computer attached gamma-camera scanner and labelled compound. With this method, 9gm Tc DISIDA-K and liver scanning was done. Those data are compared with ICG-K value of pre-and post-hepatic resectcion.
Material and method
14 dogs weighing around 10 Kg are used for control and experiment. In experimental group, functional hepatic resection ranging from 15% to 90% was done. Pre-and post-operatively ICGK and 99mTc DISIDA-K were measured.
Result and conclusion
1) Before operation ICG-K is 0.1¡¾0.023 min1 (mean¡¾S.E) and 99mTc DISIDA-K is 1.060¡¾0.067 min.
2) Relationship between ICG-K pre to post-operation versus up-graded hepatectomy can be expressed as follow. Y= -0.99X+100
3) Relationship between 99mTc DISIDA-K post-operation to pre-operative value versus up-graded hepatectomy can be shown as follow. Y=-0.80X+93
4) Likewise between 99mTc DISIDA-K and ICG-K pre-and post-operative value can be shown as follow. Y=0.79X+13
In conclusion, it can be stated that 99mTc DISIDA-K with liver scanning image are better simplified method to evaluate liver condition quantitatively and qualitatively.
